Patient Registration Medical Record Release
It is very important to fully complete this form. You may bring a list of all medications that you are currently taking (if you do not want to write them down). Please be sure to bring your eyeglasses with you. It is very important to indicate your exact “reason for visit”. This will assist us in determining your insurance benefits for a medical reason such as diabetes or glaucoma or a routine vision checkup.
If your insurance company requires you to have a referral, you must bring a copy with you. Without the referral, you may be asked to reschedule your appointment. Referrals are the responsibility of the patient. If you are unsure about your need for a referral, please call your insurance company or our office, and we will be glad to help you.

If you do not have Mozilla Firefox as an available browser, you can go to www.google.com and search Mozilla Firefox then download the program for free. If you try to open your patient portal using another browser like Internet Explorer, you may not be able to access all information.
Once logged into the portal you will see the home page where you can view and edit portions of your medical record. You can also communicate any questions or requests securely through the portal!
