REGISTRATION

We ask that you complete the attached paperwork.  Please bring your insurance card(s) and a form of photo identification with you to your appointment. At the time of your appointment we will verify your insurance benefits and coverage.  This will take some additional time. We appreciate your patience.  You are welcome to arrive a few minutes prior to your appointment time.

HEALTH HISTORY

It is very important to fully complete this form. You may bring a list of all medications that you are currently taking (if you do not want to write them down). Please be sure to bring your eyeglasses with you. It is very important to indicate your exact “reason for visit”. This will assist us in determining your insurance benefits for a medical reason such as diabetes or glaucoma or a routine vision checkup.

REFERRALS

If your insurance company requires you to have a referral, you must bring a copy with you.  Without the referral, you may be asked to reschedule your appointment.  Referrals are the responsibility of the patient.  If you are unsure about your need for a referral, please call your insurance company or our office, and we will be glad to help you.

Please contact our office at 301-805-9200 to be given your username to log in or a valid email address and we can send you a link with all the information.

Once you have received your Username you can log on.

Your password will be the first letter of your first name (uppercase) then the first letter of your last name (lowercase) followed by your six-digit date of birth.

Example: Mr. John Smith DOB: 10/15/1945
His password would be: Js101548

Just type in alanmaloufmd.ema.md directly into Mozilla Firefox browser bar. You must use Mozilla Firefox or you will not be able to utilize your patient portal properly.

DO NOT TYPE www or https before the URL above.  If you type it into a search bar you may not get our true website. If you do not have Mozilla Firefox as an available browser, you can go to www.google.com and search Mozilla Firefox then download the program for free. If you try to open your patient portal using another browser like Internet Explorer, you may not be able to access all information.

Once logged into the portal you will see the home page where you can view and edit portions of your medical record. You can also communicate any questions or requests securely through the portal!
